About The Position

Advanced Practice Provider — Primary Care Nurse Practitioner or Physician Assistant. You’ll manage your own patient panel, deliver continuity-focused care including preventive, acute, and chronic disease management, and work within an integrated care model alongside physicians, behavioral health clinicians, care managers, and community health workers.
