About The Position

You will deliver field-based care, conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. You will manage chronic disease by creating longitudinal care plans that move the needle for high-risk patients. You will collaborate across the continuum by partnering with an interdisciplinary team to ensure seamless, safe care transitions. You will prevent ER visits by treating acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Requirements

Master’s Degree (NP) with a preferred focus on Adult, Geriatric, Acute Care and/or Family.
Active, unencumbered NP license and Board Certification (ANCC or AANP).
Valid driver’s license and vehicle for local field-based travel.
Must be legally authorized to work in the United States without the need for employer sponsorship now or in the future.
Must successfully complete a pre-employment drug screening.
Must successfully complete a background check.
Must have satisfactory results on monthly FACIS checks.
