About The Position
The HealthCare Connection is seeking a dedicated and experienced Nurse Practitioner to join its School Based Health team. As part of the clinical provider team, the Nurse Practitioner delivers direct primary care services, including patient assessment, diagnosis, treatment implementation, evaluation, and ongoing monitoring of care plans. The role collaborates with physicians and healthcare staff to provide high-quality, patient-centered care and support positive health outcomes.
Requirements
- Master’s or doctoral degree in Nursing.
- Certified Nurse Practitioner licensed in the State of Ohio.
- Minimum of 5 years of experience in family medical practice.
- Board certification as a Nurse Practitioner.
- Active DEA license or ability to obtain one.
- Strong clinical decision-making and communication skills.
- Commitment to patient-centered care.
Nice To Haves
- Experience in a physician office, ambulatory care, or clinical health setting.
- Experience in primary care, family medicine, or community health.
- Bilingual skills, if applicable.
- Familiarity with electronic health record (EHR) systems.
Responsibilities
- Diagnose and treat acute and chronic conditions to provide high-quality patient care.
- Obtain and review medical, developmental, social, and family histories, and perform comprehensive physical exams.
- Order, perform, and interpret diagnostic tests and preventive screenings.
- Develop and manage treatment plans, prescribe medications, and educate patients about conditions and care.
- Promote preventive care through immunizations, screenings, and education about health, nutrition, safety, and disease prevention.
- Accurately document patient encounters, including history, exam findings, diagnoses, and care plans.
- Review and maintain patient records, including labs, imaging, medications, and immunization status.
- Coordinate care by facilitating referrals and collaborating with healthcare team members.
- Monitor patient progress, follow up on test results, and manage high-risk patient populations.
