About The Position

We are a unique, rapidly expanding primary care provider focused on transforming healthcare for seniors. As a Nurse Practitioner (NP) on our clinical operations team, you will provide direct patient care in various settings including medical centers, nursing homes, skilled nursing facilities (SNF), and home settings. Your role will involve geriatric assessments, medical history taking, physical examinations, diagnosis and treatment, developing nursing care plans, health education, and coordinating referrals. You will also participate in clinical rounds, conferences, and maintain clear documentation according to ChenMed standards. This position requires the ability to function both independently and collaboratively with other healthcare professionals, adhering to strict departmental goals, quality standards, and regulatory compliance. You will practice in accordance with a written or electronic practice agreement, consulting with management and physicians to ensure compliance with guidelines. This role may involve participation in risk and quality management programs and other meetings.

Requirements

ARNP or similar advanced degree in Nursing required
Current Nurse Practitioner Certification in the State of practice required
Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment
Demonstrated record of consistently achieving clinical performance metrics in current role
Strong Critical Thinking and problem-solving skills
Excellent communication and interpersonal skills
Time management skills with the ability to work well under pressure
Must be caring and empathetic and have great listening skills
Must be detail-oriented, and able to pay close attention to patient charts, medications, and follow-up on details of patient care
Basic computer skills and some knowledge of Microsoft Office Suite

Nice To Haves

Board certification by AANP or ANCC is preferred but may be required for certain States
A minimum of 3-5 years of clinical experience preferred, but willing to consider strong new graduate

Responsibilities

Independently assess acute and non-acute clinical problems.
Perform and document physical assessments and patient histories.
Analyze trends in patient conditions and develop, document, and implement patient management plans.
Assist in the development of the nursing plan of care.
Provide appropriate patient/family/significant other counseling and education.
Plan patient care based on in-depth knowledge of the specific patient population and/or protocol, anticipating and identifying physiological and/or psychological problems.
Consider the patient’s cultural background, level of understanding, personality, and support systems.
Serve as a patient advocate.
Write admission, transfer, and discharge orders.
Order and interpret appropriate laboratory and diagnostic studies.
Order appropriate medication and treatments.
Refer patients for consultation when indicated (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary).
Document through in-depth progress notes and summaries.
May perform invasive procedures independently upon completion of documented competency.
Participate in patient care rounds and conferences.
Communicate patient management strategies to members of the patient care team.
Collaborate with members of the multidisciplinary team to ensure patient management strategies are successful.
Recognize situations requiring immediate physician attention and initiate life-saving procedures when necessary.
Use advanced communication skills to problem-solve complex situations and improve processes and patient service.
Collaborate with other multidisciplinary team members to analyze and evaluate current health care delivery systems and implement new practice patterns.
Participate in outside activities that enhance personal and professional growth and development.
Initiate arrangements and write orders for SNF discharges and complete appropriate paperwork.
Work collaboratively with physicians, nurses, PT, social workers, family, and key caregivers to transition the patient to a lower level of care as soon as medically appropriate.
Introduce self to patient/family and explain nurse practitioner role.
Facilitate patient/family conferences to review treatment goals, optimize resource utilization, provide family education, and identify post-SNF needs.
Enhance collaborative relationships to maximize the patient’s/family’s ability to make informed decisions regarding goals of care, palliative care, and hospice.
Facilitate discharge to appropriate level of care and use preferred providers when additional services are required.
Prescribe medication to patients based on the State of practice.
Perform other duties as assigned and modified at manager’s discretion.
