Requirements
- Minimum of 2 years’ of experience
- Must have National Certification
- Hold or be eligible to obtain an Arizona License and DEA
- Must have a Level One Fingerprint Clearance card.
- Undergo drug testing prior to employment.
- Maintain vaccination status (i.e., as recommended by CDC and/or other public health agencies) to include an Influenza vaccination.

- Prefer working at an FQHC, CHC, or PCMH facility
