About The Position

The Nurse Practitioner consults with and educates patients and families about disease processes, self-care techniques, and end-of-life care. The role works with the interdisciplinary group (IDG) to develop patient and family plans of care and collaborates with attending, consulting, and hospice physicians to support palliative care. The Nurse Practitioner monitors patient needs and outcomes, evaluates patient and family responses to care, submits documentation on time, and ensures compliance with applicable laws, regulations, and policies.

Requirements

• Graduate of a Baccalaureate School of Nursing.
• Current state license as a Registered Nurse and as an Advanced Practice Nurse (APN).
• Current driver's license, reliable transportation, and ongoing valid and current auto liability insurance.
• Two years of experience as an APN in a clinical care setting.
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.

Nice To Haves

• Experience in hospice or home health is preferred.

Responsibilities

• Consult with and educate patients and families regarding disease processes, self-care techniques, and end-of-life care.
• Consult with and assist the IDG in developing patient and family plans of care to ensure quality care.
• Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with the hospice physician.
• Inform physicians and other personnel of changes in patient needs and outcomes of interventions.
• Evaluate patient and family responses to care.
• Submit all documentation in a timely manner.
• Perform other duties as required.
• Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
