The Palliative Care Nurse Practitioner, under clinical supervision of a licensed physician, is responsible for diagnosis, treatment and management of patients who are dealing with a progressive illness and provides care in keeping with parameters established for nurse practitioners. As a primary care provider, the nurse practitioner will provide services appropriate for a hospice and palliative care specialist. The palliative care nurse practitioner will collaborate with the covering physicians/medical as appropriate. The palliative care nurse practitioner may provide hospice and/or palliative care services in a variety of settings including hospitals, clinics, home care, and nursing homes and assisted living while consistently demonstrating the Hospice Savannah competencies, values, and service excellence standards. The palliative care nurse practitioner is accountable to and evaluated by the Steward Center medical director.
