About The Position

Reporting to the Director, Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens) as determined by community and program needs. Depending on community and team requirements, the position may include first on-call support. Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation.

Requirements

  • Master of Nursing - Nurse Practitioner (MN-NP)
  • Valid Class 5 driver's license
  • Advanced Cardiac Life Support Certificate (ACLS)
  • Certification in Primary Care Nurse Practitioner or equivalent program required
  • Basic Life Support Level C Certificate (BLS-C)
  • Registered/eligible for registration with the CRNS as RN (NP)

  • Knowledge and skill in quality improvement and patient safety
  • Knowledge in chronic disease prevention and management

Nice To Haves

  • Three (3) years experience working as a Registered Nurse
  • Experience as RN (NP) in a Primary Health Care setting preferred
