This role is perfect for someone who is dedicated to providing comfort, dignity, and high-quality palliative care to patients and their families during life’s most meaningful moments. Requirements
- Current NP or CNS certification in the state of Indiana
- Valid driver’s license, auto insurance, and reliable transportation
Nice To Haves
- Minimum 5 years of nursing experience
Responsibilities
- Conduct required face-to-face hospice visits and report findings to the Hospice Medical Director
- Develop, coordinate, and document patient plans of care
- Collaborate with attending physicians and IDG members
- Prescribe medications (including controlled substances) within scope and delegation
- Order diagnostics and treatments as appropriate
- Educate patients, families, staff, and community on hospice and palliative care
- Participate in IDG and QAPI meetings
- Maintain accurate clinical documentation for compliance and reimbursement
- Supervise and evaluate home health aides as appropriate
- Complete required training modules
