About The Position

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Join Senior Care Partners, PACE as a Nurse Practitioner and play a vital role in delivering compassionate, coordinated primary care to frail older adults. In this role, you’ll build meaningful relationships with participants and their families while working alongside an interdisciplinary team dedicated to providing high-quality, patient- and family-centered care. Under the direct supervision of the Medical Director/Primary Care Physician, the Nurse Practitioner provides primary medical care to PACE participants in accordance with organizational policies, the Nurse Practitioner Collaborative Practice Agreement, and applicable standards of practice.

Requirements

Master of Science in Nursing (MSN) or Nurse Practitioner degree.
Graduation from an accredited Nurse Practitioner educational program.
Current Michigan Registered Nurse license.
Current Michigan Nurse Practitioner license.
Minimum of one year of experience working with a frail elderly population.
Strong written and verbal communication skills.
Excellent judgment, discretion, and commitment to confidentiality.
Proficiency with Microsoft Office, electronic systems, and related applications.
Proven leadership abilities and the ability to collaborate effectively within a team environment.
Demonstrates the core values of belonging, ownership, partnership, and excellence.
Current valid Michigan driver’s license, reliable transportation, and required automobile insurance.
Current AHA CPR certification, or willingness and ability to obtain certification through Senior Care Partners, PACE.
Ability to practice within the appropriate scope of authority.
Ability to successfully complete position-specific competencies established by Senior Care Partners, PACE and approved by CMS before working independently.
Knowledge of current medical practices, procedures, and applicable long-term care laws, regulations, and guidelines.
Ability to make sound independent clinical decisions when circumstances require.
Strong interpersonal skills and the ability to communicate respectfully with participants, families, staff, government representatives, visitors, and community partners.
Ability to plan, organize, implement, and support organizational goals, policies, procedures, and quality-care initiatives.
Patience, tact, enthusiasm, and a positive disposition when working with participants and families in challenging circumstances.
Commitment to the philosophy and mission of Senior Care Partners, PACE.
Must be fully vaccinated or willing to become vaccinated against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.
Must meet CMS competency standards prior to working independently.
Must be willing to work beyond normal hours.
Comfortable working in varied home environments and all weather conditions.

Responsibilities

Conduct comprehensive histories and physicals for new referrals.
Perform quarterly and clinically indicated interval assessments for established participants.
Evaluate and treat episodic illnesses and changing health needs.
Participate actively in the Interdisciplinary Team (IDT) and participant care planning.
Collaborate with physicians, nurses, therapists, social workers, caregivers, families, and other professionals to coordinate quality care around the clock.
Monitor participant conditions, document changes accurately, and communicate clinical concerns promptly.
Develop and implement appropriate medical orders and determine when specialty consultation is needed.
Serve as a clinical resource to the health care team and help ensure continuity and quality of care.
Support participant and family decision-making through a patient- and family-centered approach.
Provide coverage and assistance across Senior Care Partners, PACE clinics as needed.
Monitor clinical supplies and communicate restocking needs.
Participate in an on-call rotation and respond to urgent or emergency situations when required.
