About The Position

Community Health is a multi-site FQHC serving 80% of the primary care needs in central Vermont. Our mission is to service our patients with quality, efficiency, accessibility and equity. As the largest FQHC in northern New England, we have a large footprint that provides us with the opportunity to influence the evolving, local, state and regional health care environment. Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.

Requirements

  • 3-5 years Express / Urgent Care Practice Experience.
  • Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
  • Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.
  • Strong inpatient skill set.
  • Quality-focused team player.
  • Excellent organizational, time management and problem-solving skills.
  • Ability to function effectively as part of a multi-disciplinary team.
  • Effective oral and written communication skills.
  • Commitment to providing exceptional patient care.

Responsibilities

  • Obtains complete medical history and physical examination on patients.
  • Reviews medical records.
  • Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
  • Establishes presumptive diagnosis.
  • Orders appropriate laboratory and diagnostic procedures.
  • Develops a therapeutic plan for patient.
  • Sutures wounds.
  • Prepares written prescription orders for drugs and controlled substances.
  • Instructs patients and family regarding medications and treatment instructions.
  • Provides recommendations regarding health and illness prevention.
  • Recommend community resource to meet patient/family needs.
  • Performs independent evaluations and treatment procedures in emergency situations.
  • Provides monitoring and continuity of care between physician visits.
