About The Position

Silverado Hospice is seeking a compassionate and skilled Nurse Practitioner (NP) to join their team in Orange County. This role is crucial for providing advanced nursing care, supporting patients and their families through end-of-life journeys, and ensuring compliance with regulatory standards. The NP will be instrumental in delivering dignified, patient-centered care and contributing to the smooth operation of the hospice program. Requirements

Graduate of an accredited school of Advanced Practice Registered Nursing.
MSN or Doctoral Degree required.
Current Advanced Practice Registered Nurse License in the state of employment.
End-of-Life Nursing Education Program (ELNEC) – all 8 modules completed within 18 months of hire.
Valid driver’s license, good driving record, and reliable transportation.
Ability to lift up to 50 pounds.
Must clear criminal background check, physical, and drug screening.
Comfortable working outside regular hours as needed.

Nice To Haves

Certification in Hospice and Palliative Nursing preferred.

Responsibilities

Perform comprehensive nursing assessments for hospice patients.
Assess signs and symptoms of discomfort, including pain, and take prompt, appropriate action.
Initiate and manage individualized plans of care for patients and families.
Provide direct face-to-face visits for referred and existing patients.
Communicate with physicians regarding patient updates and care plans.
Maintain knowledge of state and federal regulations and JCAHO standards (if applicable).
Participate in Quality Assessment and Performance Improvement (QAPI) programs.
Ensure all services comply with hospice standards and regulatory requirements.
