About The Position

Silverado Hospice is seeking a compassionate and skilled Nurse Practitioner (NP) to join their team in Orange County. This role is crucial for providing advanced nursing care, supporting patients and their families through end-of-life journeys, and ensuring compliance with regulatory standards. The NP will be instrumental in delivering dignified, patient-centered care and contributing to the smooth operation of the hospice program. Requirements

  • Graduate of an accredited school of Advanced Practice Registered Nursing.
  • MSN or Doctoral Degree required.
  • Current Advanced Practice Registered Nurse License in the state of employment.
  • End-of-Life Nursing Education Program (ELNEC) – all 8 modules completed within 18 months of hire.
  • Valid driver’s license, good driving record, and reliable transportation.
  • Ability to lift up to 50 pounds.
  • Must clear criminal background check, physical, and drug screening.
  • Comfortable working outside regular hours as needed.

Nice To Haves

  • Certification in Hospice and Palliative Nursing preferred.

Responsibilities

  • Perform comprehensive nursing assessments for hospice patients.
  • Assess signs and symptoms of discomfort, including pain, and take prompt, appropriate action.
  • Initiate and manage individualized plans of care for patients and families.
  • Provide direct face-to-face visits for referred and existing patients.
  • Communicate with physicians regarding patient updates and care plans.
  • Maintain knowledge of state and federal regulations and JCAHO standards (if applicable).
  • Participate in Quality Assessment and Performance Improvement (QAPI) programs.
  • Ensure all services comply with hospice standards and regulatory requirements.
