The Nurse Practitioner will consult with and educate patients and their families regarding their disease process, self-care techniques, and end-of-life care. This role involves consulting with and assisting in the Interdisciplinary Group's (IDG) development of a patient/family plan of care to ensure quality care. The Nurse Practitioner will interact with attending and consulting physicians regarding palliative care, in collaboration with the hospice physician, and inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care. Timely submission of all documentation and performance of other duties as required are essential. The role also ensures overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
