About The Position

Circadia Health is a growth-stage healthcare AI company focused on preventing avoidable hospitalizations and transforming senior-care operations. Its Circadia Intelligence Platform uses contactless sensing, predictive models, and enterprise integrations to support post-acute care. The company’s technology reaches more than 40,000 post-acute patients daily, and Circadia Health is headquartered in El Segundo, California. The Nurse Practitioner will provide on-site care at partner facilities in Central New Jersey, using predictive health signals to identify patients who need timely intervention and helping facility teams translate data into care. The role combines direct patient care with staff training, workflow integration, and collaboration with clinical operations and product teams. This is a full-time, in-field position with Monday–Friday presence at partner facilities. The NP will typically complete high-volume rounds across designated skilled nursing facilities, including new admissions, facility-flagged residents, and patients identified by Circadia alerts. Sites are generally within a 30–40-mile radius; mileage and lodging for sites outside that radius are covered per policy. Success is measured through visit volume, facility engagement, acknowledgement time, documentation, workflow adoption, and trends in falls and readmissions. Documentation is completed using AI-powered tools within 24 hours of each encounter.
