This role is for a Nurse Practitioner who will perform comprehensive health assessments, including obtaining history, performing physical examinations, completing medicine reviews, assessing home safety and psychosocial needs, and fulfilling documentation for STAR and HCC measures. The Nurse Practitioner will also obtain and document all pertinent information in patient charts and case management notes on the same day as the patient is seen, including medications, end-of-life preferences, and drivers of preventable hospital admissions. They will instruct patients, caregivers, and families to call prior to accessing medical care, except in life-threatening emergencies, and maintain patient safety and confidentiality using universal precautions. The Nurse Practitioner will prescribe/order procedures, treatments, and medications within their scope of practice and develop a high level of trust and intimacy with patients and families.

Requirements

  • CA Licensed Nurse Practitioner
  • 1+ Years of Experience with Primary Care

Responsibilities

  • Perform comprehensive health assessment
