Requirements

• Valid/active NYS RN and NP licensure
• Masters degree in a Nursing
• Computer proficiency in electronic health records, health information technology and health informatics required
• Promote the organization’s code of conduct, safety rules, and adherence to all company policies
• Carry out job responsibilities in an ethical, effective, comprehensive and professional manner
• Cooperate, work, and communicate with staff, supervisors, management, and patients in a professional manner
• Display compassion and work well in emergent and stressful situations
• Demonstrate and enforce standards for safe, quality patient care, setting high standards by example
• Must be keenly aware of the importance of confidentiality in all aspects of the position

• Experience in family practice preferred, but not required

Responsibilities

• Provide comprehensive patient care, including diagnosis, treatment, and referrals
• Deliver care within your scope of practice, collaborating with physicians as needed
• Maintain accurate, compliant medical records in accordance with all regulations
• Participate in quality improvement initiatives, peer review, and continuing education
• Support development and monitoring of healthcare goals and quality metrics
• Engage in community outreach, helping to promote the role of the Nurse Practitioner
• Collaborate with leadership and medical staff to improve care delivery and efficiency
• Contribute to Patient-Centered Medical Home (PCMH) initiatives
