About The Position
A growing palliative care organization is seeking an experienced Nurse Practitioner to provide compassionate, high-quality care to patients with serious illnesses and complex chronic conditions throughout Ocean County and surrounding areas. This role offers the opportunity to work within a collaborative interdisciplinary team while making a meaningful impact on patients and families through comprehensive symptom management, advance care planning, and patient-centered care.
Requirements
- Active New Jersey Nurse Practitioner license
- National board certification and DEA registration
- Minimum 2 years of Nurse Practitioner experience preferred
- Experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management preferred
- Strong communication and interpersonal skills
- Ability to work independently while collaborating within a multidisciplinary team
Nice To Haves
- Hospice & Palliative Care certification (ACHPN)
- Experience with home-based care, telehealth, or value-based care models
- Bilingual skills are a plus
Responsibilities
- Conduct comprehensive palliative care assessments and follow-up visits
- Manage patients with serious illness and complex chronic conditions
- Facilitate goals-of-care conversations and advance care planning discussions
- Develop and implement individualized care plans
- Collaborate with physicians, specialists, hospitals, skilled nursing facilities, and community providers
- Provide symptom management and patient/family education
- Document patient encounters accurately and in a timely manner
- Participate in interdisciplinary team meetings and care coordination activities
