About The Position

Silverado Palliative Coastal Valleys is hiring a Nurse Practitioner to serve Los Angeles County. The role provides advanced nursing care to palliative patients, supports patients and families through end-of-life journeys, and helps ensure regulatory compliance. The clinician works with an interdisciplinary team to deliver dignified, patient-centered care and support the operation of the Palliative program. This is a per-diem (PRN) position with a Monday–Friday schedule of 8:00 a.m.–5:00 p.m.; shifts vary as needed to meet business needs. The role requires face-to-face visits and may involve working outside regular hours as needed. The anticipated pay range is $75–$80 USD.

Requirements

Graduate of an accredited school of Advanced Practice Registered Nursing.
MSN or doctoral degree required.
Current Advanced Practice Registered Nurse license in the state of employment.
Complete all 8 End-of-Life Nursing Education Program (ELNEC) modules within 18 months of hire.
Valid driver’s license, good driving record, and reliable transportation.
Ability to lift up to 50 pounds.
Must clear criminal background check, physical, and drug screening.
Comfortable working outside regular hours as needed.

Nice To Haves

Certification in Hospice and Palliative Nursing preferred.

Responsibilities

Perform comprehensive nursing assessments for palliative patients.
Assess signs and symptoms of discomfort, including pain, and take prompt, appropriate action.
Initiate and manage individualized plans of care for patients and families.
Provide direct face-to-face visits for referred and existing patients.
Communicate with physicians regarding patient updates and care plans.
Maintain knowledge of state and federal regulations and JCAHO standards, if applicable.
Participate in Quality Assessment and Performance Improvement (QAPI) programs.
Ensure services comply with hospice standards and regulatory requirements.
