Per diem coverage will be 7:00am - 3:00pm and 3:00pm - 11:00pm, including every other weekend and one summer holiday / one winter holiday rotation. Service Areas: Greater Nashua and surrounding communities.

Requirements

• Master's or postgraduate NP degree
• Active RN and APRN licenses (NH)
• Unrestricted DEA and Controlled Substance License
• Eligible for hospital privileges
• BLS certification
• Strong communication and teaching skills
• Satisfactory background and driving records
• Active driver’s license and auto insurance

Nice To Haves

• Existing local hospital privileges
• Advanced Hospice & Palliative Care Certification
• Expertise in pain/symptom management and palliative pharmacology
• Experience supporting psychosocial needs of terminally ill patients and families

Responsibilities

• Provide hospice and palliative care visits and consultations
• Deliver expert pain and symptom management
• Serve as a resource for community clinicians
• Support development and maintenance of clinical standards
• Participate in on-call rotation
• Educate and support hospice staff using evidence-based practice
• Build strong relationships with community providers
• Complete timely evaluations, documentation, and coding
• Maintain ongoing professional education
