About The Position

We are recruiting Nurse Practitioners for a fully remote, full-time W-2 direct provider role with a value-based healthcare company launching its first clinical teams across multiple states. The Delaware market is one of our most urgent needs right now. The mission of this organization is simple: turn the home into the engine of care. They serve medically complex Medicaid and Dual Eligible members, frail elderly individuals, and people with multiple chronic conditions who too often have no clinical pathway when early warning signs show up. They exist to be that proactive clinical bridge, catching changes early, intervening fast, and keeping patients safely at home.

Requirements

  • Active Delaware NP license — candidate must currently live in Delaware OR SURROUNDING STATES and licensed in DE.
  • FNP or AGNP board certification required. General NP only is not sufficient.
  • Minimum 3 to 5 years of independent NP practice.
  • True primary care or longitudinal chronic disease management experience. Urgent care only or episodic care backgrounds are not a fit.
  • Significant telehealth experience that is current and active, not just COVID era. Must be comfortable making clinical decisions based solely on a video visit with no hands-on access, no vitals, and no remote monitoring equipment.
  • Value-based care experience or a demonstrated understanding of the philosophy. Fee-for-service mindset is a red flag.
  • Familiarity with HCC coding, HEDIS measures, and STAR ratings in day-to-day practice. Candidates who cannot give specific examples of how they have used these in practice will not advance.
  • Active DEA and full prescriptive authority.
  • Medicare and Medicaid provider number preferred.
  • Entrepreneurial mindset and comfort with a startup environment.
  • This must be their primary role. Candidates with multiple active employer commitments that would compete with a Monday through Friday eight to five schedule are not a fit.

Responsibilities

  • Conduct virtual patient assessments, medication management, care planning, and follow-up through video visits.
  • Coordinate with PCPs, specialists, managed care organizations, and community partners.
  • Lead advance care planning and goals-of-care conversations with patients and families.
  • Support HCC coding, risk adjustment, and HEDIS and STAR quality gap closure through documentation.
  • Participate in a shared rotating 24/7 after-hours triage component.
