About The Position

As an essential part of our Ambulatory Care Network (ACN), you will join a multidisciplinary team in an outpatient care setting to provide comprehensive health care and enhance the community’s access to outpatient services and improve coordination with the Hospital’s inpatient programs. The Washington Heights Clinic has transformed how we provide care to the Washington Heights community through its successful pilot of the Interdisciplinary Care Team, core to the Patient-Centered Medical Home model. All members of the care team—Nurses, Nutritionist, Care Manager, Community Health Worker, Patient Financial Assistants, Social Worker, Medical Assistants, and Physicians collaborate to improve the health and well-being of our patients. Use your expertise and passion for educating, providing services and care to patients of all ages and empowering them to stay healthy. Requirements

  • Master’s degree in Advanced Practice
  • BSN
  • Current NYS NP & RN License
  • NYS NP certification (FNP)
  • BLS from American Heart Association
  • Strong communication and interpersonal skills
  • All roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

Nice To Haves

  • Minimum three years of experience as a Nurse Practitioner in Internal Medicine
  • Outpatient experience
  • Bilingual - English/Spanish
