About The Position
The Nurse Practitioner will round at assigned facilities and manage the acute, subacute, and primary care needs of a fragile population. This role involves close collaboration with the OnSite team, nursing home staff, residents, and families. The practitioner will proactively follow up with patients, assess the efficacy of recommended interventions, and provide education on best practices. A key aspect of this role is integrating various interventions, with an emphasis on non-pharmacological treatments whenever possible.
Requirements
- Master of Science in Nursing with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification
- Active License in South Carolina
- DEA registration
- 1+ year experience
- Excellent communication and documentation abilities.
- Strong patient assessment skills.
- Strong, ambitious, patient-centric work ethic.
