When patients see their specialists at Upperline, they are provided access to a comprehensive care team that ensures patients receive the care they need. Our providers coordinate patients’ care among a team of specialists - physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers for integrated treatment that addresses patients’ immediate and long-term health needs. Upperline Health is based out of Nashville, TN and has clinics in Alabama, California, Florida, Georgia, Indiana, Kentucky, and Tennessee. We put patients at the center of value-based care.
