We are seeking a compassionate and skilled Primary Care Nurse Practitioner to join our medical group on a full time basis. The ideal candidate will provide comprehensive primary care services to our senior residents, ensuring they receive the highest quality of medical care. This role involves collaborating with other healthcare professionals to develop and implement individualized care plans, promoting wellness, and managing chronic conditions. The candidate will be expected to work 3 days at Applewood Village Freehold NJ and 2 days at Pines Village Whiting NJ.

Requirements

Master's degree in Nursing from an accredited program.
Current Nurse Practitioner license in New Jersey.
Minimum of 5 years of experience in a primary care or geriatrics setting.
Excellent clinical skills and knowledge of geriatric care.
Strong communication and interpersonal skills.
Ability to work collaboratively in a team environment.
Compassionate and patient-centered approach to care.

Nice To Haves

Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) preferred.

Responsibilities

Conduct thorough assessments and physical examinations of senior residents.
Diagnose and manage acute and chronic health conditions.
Develop, implement, and monitor individualized care plans.
Prescribe medications and treatments as necessary.
Provide health education and counseling to residents and their families.
Collaborate with physicians, nurses, and other healthcare team members.
Maintain accurate and up-to-date medical records.
Participate in care team meetings and contribute to quality improvement initiatives.
Ensure compliance with all regulatory and organizational policies and procedures.
