About The Position

Kinetic Medical Group (KMG) is seeking a Full-Time Nurse Practitioner specializing in Physical Medicine & Rehabilitation to serve patients in a skilled nursing and rehabilitation facility in Belmont, NC. This role offers significant clinical autonomy, allowing the NP to act as a trusted clinical leader, collaborating with various teams to improve patient recovery, function, and quality of life. Responsibilities

Provide comprehensive medical care to geriatric and rehabilitation patients within the SNF setting
Evaluate and manage patients' rehabilitation and physical medicine needs
Develop and monitor individualized treatment plans
Collaborate closely with nursing, physical therapy, occupational therapy, speech therapy, administration, and families
Identify and address medical issues before they become emergencies
Participate in preventative care and quality-improvement initiatives
Help reduce avoidable hospitalizations and improve patient outcomes
Communicate effectively with patients, families, and the interdisciplinary care team
Serve as a clinical resource and trusted partner to the facility
Practice with a high degree of autonomy while having support from the KMG team
