About The Position

Kinetic Medical Group (KMG) is seeking a Full-Time Nurse Practitioner specializing in Physical Medicine & Rehabilitation to serve patients in a skilled nursing and rehabilitation facility in Belmont, NC. This role offers significant clinical autonomy, allowing the NP to act as a trusted clinical leader, collaborating with various teams to improve patient recovery, function, and quality of life. Responsibilities

  • Provide comprehensive medical care to geriatric and rehabilitation patients within the SNF setting
  • Evaluate and manage patients' rehabilitation and physical medicine needs
  • Develop and monitor individualized treatment plans
  • Collaborate closely with nursing, physical therapy, occupational therapy, speech therapy, administration, and families
  • Identify and address medical issues before they become emergencies
  • Participate in preventative care and quality-improvement initiatives
  • Help reduce avoidable hospitalizations and improve patient outcomes
  • Communicate effectively with patients, families, and the interdisciplinary care team
  • Serve as a clinical resource and trusted partner to the facility
  • Practice with a high degree of autonomy while having support from the KMG team
