About The Position
This is a part-time position for a Nurse Practitioner in a Long Term Care facility in Lima, OH. The facility is seeking a dedicated Nurse Practitioner experienced in Primary Care to provide comprehensive nursing and medical care to patients. The role requires adherence to organizational standards and policies, and demonstration of adequate knowledge, judgment, and technical skills to ensure care plans and facility procedures are met.
Requirements
- Valid NP license in the state of OH
- Ok with nursing home setting

Responsibilities
- Managing all Primary Care related consults
- History and physical exam
- Medication review
- Prescribe medication and drugs
- Check in with patients who have acute issues and establish a follow-up plan
- Reach an informed diagnosis based on medical knowledge and the patient's medical history
