About The Position
The Nurse Practitioner (VSP) applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner (VSP) works on problems of diverse scope and complexity ranging from moderate to substantial. This is a variable service provider NP/PA (APP) role with an average of 20-30 hours per week. Licensure preferred in TX.
Requirements
- Master's in science in Nursing
- Active, unrestricted NP Licensure preferred in TX
- 2+ years of experience
- Self-provided internet service must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested.
- Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Responsibilities
- Call Humana members to discuss open care gaps and placing needed orders (specifically women who had a fracture in the last 6 months)
- Create and register a patient record within Athena
- Verify benefits
- Gather medication, medical and surgical history
- Coordinate management of members social needs to help facilitate gap closure – providing transportation services.
