About The Position

The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking a Nurse Practitioner to join our team of dedicated specialists working in the Medicine Department. Now hiring for the 11:00am-11:00pm or 3:00pm-11:00pm Shift. In this role working alongside caring, dedicated, and experienced Medical leadership, the Nurse Practitioner will report to the Medical Director, or the supervisory Attending Physician, and is responsible for the delivery of compassionate and quality medical care of assigned residents in coordination with other members of the health care team.

Requirements

Graduate of an accredited nurse practitioner program
Active license as an NP in New York State, with prescriptive privileges
NYS Board Certified is required
Proficient in SigmaCare or another EMR (Electronic Medical Record) software

Nice To Haves

Prior experience with the geriatric population preferred
Two (2) or more years of experience in a sub-acute or long term care facility

Responsibilities

Examining residents and delivering quality care in varied settings including subacute, long term, rehab and outpatient care
Assist in resident/patient/family education
Re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts
Review all diagnostic testing results and make recommendations to findings
Educate and supervisor rotating medical and physician assistant students
