Requirements

  • Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
  • One to two years clinical experience in role of nurse practitioner required
  • Minimum one year in geriatric specialty
  • NYS RN/NP License, Registration, DEA certificates

Nice To Haves

  • Previous clinical RN experience in acute, subacute and/or long term care setting preferred
  • Prior experience in managed care a plus

Responsibilities

  • Provide comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self-management education & tools to better manage patient’s multiple conditions.
  • Provide and coordinate primary care services to an assigned at-risk panel of Elderplan members residing in the community as part of the Interdisciplinary Co-Management Program.
  • Ensure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives.
  • Act as a clinical resource to the member, family, and interdisciplinary team.
  • Provide and/or coordinate primary clinical care services for assigned high-risk members and facilitate supportive care during transitions from acute or sub-acute care settings.
  • Act as a team player, member’s advocate, and clinical lead while working closely with the Interdisciplinary Team to coordinate member’s care and optimize health outcomes.
  • Act as a resource and contact for the member/designated representative regarding all aspects of the member’s individualized care plan and disease process.
