About The Position

This is a clinical position designed for an NP or PA who wants to practice at the top of their training in a supportive, collaborative environment. You will co-manage a defined panel of geriatric patients, provide outpatient primary care visits in a geriatric-focused clinic, and conduct rounds in Assisted Living and Independent Living communities. You will also participate in Skilled Nursing Facility (SNF) care alongside the physician, perform post-hospital / post-SNF transitional care follow-ups, and engage in medication reconciliation, goals-of-care discussions, complex care planning, and comprehensive Annual Wellness Visits. Collaboration with facility staff, families, and interdisciplinary teams is key. You will be integrated into the physician’s practice, not functioning independently without support. This role offers direct mentorship from a fellowship-trained geriatrician, providing active collaboration, real-time feedback, and meaningful clinical development. It features a true continuity model where you will see patients in clinic, assisted/independent living, skilled nursing, and after hospital discharge, providing rare exposure to the full arc of geriatric care. You will manage complex medicine including polypharmacy, frailty syndromes, dementia care, high-utilizer patients, advanced care planning, and chronic disease optimization. For those interested, a long-term leadership trajectory may include involvement in clinic management, clinical operations strategy, program expansion, and interdisciplinary team development.

Requirements

Board-certified NP or PA
Interest in geriatrics
Comfortable in multiple care settings
Strong communicator with patients and families
Detail-oriented and thoughtful clinician
Values mentorship and team-based practice

Nice To Haves

Experience in geriatrics preferred
New graduates with strong clinical rotations in geriatrics may be considered
