About The Position

The Nurse Practitioner completes Comprehensive Health Assessments and Health Risk Assessments for Medicare and Medicaid members in their homes and residential settings. The role centers on preventive care, early detection, and closing care gaps rather than managing ongoing treatment plans or maintaining a patient panel. Visits are typically 60 minutes and may take place in homes or skilled nursing facility settings. The position offers autonomous, independent field work and one-on-one patient interaction, with a local territory and limited travel. The full-time schedule is typically Monday–Friday, 8:00 AM–5:30 PM, with flexibility available; assessments may be conducted Sunday–Saturday. There is no on-call requirement. Compensation is a base salary of $103,700–$123,500, based on experience and market, with a monthly uncapped incentive plan, a year-end bonus opportunity of up to $25,000, an average annual incentive payout of $25,500, and mileage reimbursement. The employer describes itself as a national leader in in-home clinical assessments for Medicare and Medicaid members, aiming to improve outcomes, reduce healthcare costs, and identify chronic conditions that may go undiagnosed in traditional settings.
