Duo Health is a medical group dedicated to transforming care for people living with chronic kidney disease (CKD). Through our Health Mobilization™ platform, we bring together multidisciplinary care teams, community nephrologists, and local healthcare partners to deliver whole-person care addressing each patient’s medical, behavioral, and social needs through one coordinated experience. Our clinical teams including Nurse Practitioners, Social Workers, Care Navigators, and other healthcare professionals work side by side to provide personalized, high-touch care to a focused patient panel. We believe strong relationships, proactive care management, and true interdisciplinary teamwork are key to achieving better outcomes and improving quality of life for the patients we serve. Duo Health is committed to building an inclusive workplace that reflects the communities we serve. We welcome clinicians and healthcare professionals from diverse backgrounds who are passionate about making a meaningful difference for patients living with complex chronic conditions. As a Nurse Practitioner at Duo Health, you’ll be a core member of an interdisciplinary care team, providing comprehensive medical care and chronic disease management to a focused panel of patients living with CKD and other complex conditions. Your work will center on preventive care, chronic disease management, patient education, and care coordination all through a value-based, relationship-centered approach. You’ll work closely with physicians, Social Workers, Care Navigators, specialists, and community resources to ensure care is coordinated, connected, and centered around the whole person. Patients are seen in their homes, virtually, or by phone, giving you the time and context to build meaningful relationships and gain a deeper understanding of each patient’s unique health needs.
