About The Position
Tembo Health is a digital health company focused on improving care for older adults, particularly in dementia care management through programs like CMS GUIDE and virtual urgent care. This role is for the first full-time Nurse Practitioner hire supporting the Illinois market. The Nurse Practitioner will lead the initial steps in diagnosing dementia, supporting patients and families throughout the process, and assisting the care navigation team with medical escalations. The position involves working closely with Tembo's Head of Medical to refine diagnostic workflows and develop a new specialty care model for dementia patients. The role offers a high-visibility, high-impact opportunity within a rapidly growing, venture-backed company.
Requirements
- Family or Adult-Gerontology Nurse Practitioner with a minimum of 1 year of experience.
- Licensed in Illinois.
- Ability to be licensed to practice virtual/outpatient care in any state.
- Must not be excluded from participating in any state or federal healthcare programs.
- Passion for caring for patients with dementia.
- Comfort with learning new technology systems.

Responsibilities
- Conduct virtual assessments of patients with suspected dementia diagnoses, including baseline cognitive assessments and ordering laboratory testing as needed.
- Perform medication reviews for patients with dementia, providing recommendations to reduce high-risk medications and initiate symptom-management medications where appropriate.
- Develop comprehensive care plans in collaboration with care navigators.
- Coordinate care with care navigators, primary care teams, external care providers, and specialists.
- Support other clinical needs as they arise, including urgent care or other geriatric practice needs as the practice evolves.
