Requirements

  • Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
  • Active, unencumbered NP license and Board Certification (ANCC or AANP).
  • Valid driver’s license and vehicle for local field-based travel.
  • This position has been designated as safety-sensitive. As such, the employee must be able to perform job duties in a manner that ensures the safety of themselves, coworkers, patients, and the public. The role requires full cognitive and physical functioning at all times.
  • Employees in safety-sensitive positions are subject to drug and alcohol testing, including for substances that may impair judgment or motor function, in accordance with applicable federal and state laws and company policy.

Responsibilities

  • Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
  • Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
  • Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
  • Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
