About The Position

The Upper Grand Family Health Team (UGFHT) offers the opportunity to join an innovative, community-based primary health care team serving the Fergus, Elora and Arthur areas. We are looking for a talented team player to join us as we continue to provide a model of excellence in rural primary health care. The Nurse Practitioner (NP) is a vital part of the UGFHT, working collaboratively with physicians and other interdisciplinary health care providers to advance the organization’s vision of improving the health of our community through equitable access to high-quality primary health care services. In this role, the NP provides comprehensive, patient-centred care across the lifespan, demonstrating advanced clinical expertise in the assessment, diagnosis, and management of acute and chronic conditions. The NP also plays a key role in health promotion, disease prevention, and supporting patients in achieving optimal health and wellness. The Nurse Practitioner is responsible for working within the full scope of practice set out by the College of Nurses of Ontario covering the domains of: direct clinical practice, education, and leadership. Key elements of the role include the health assessment and therapeutic management of patients, addressing patient outcomes specifically and globally; patient safety; and quality of care delivery. The Nurse Practitioner will work with the multidisciplinary team and community partners to provide integrated services to meet the health needs of their patients.

Requirements

  • Current registration with the College of Nurses of Ontario
  • Certificate of Competence with Primary Care Nurse Practitioner Extended Class certification
  • Able to function as a team player working collaboratively with physicians, other FHT staff and community partners
  • Excellent communication and problem solving skills
  • Sound organizational, teaching and leadership skills
  • Proficiency in the use of computer and communication system knowledge
  • Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery
  • High degree of flexibility

Nice To Haves

  • RNAO / NPAO membership preferred
  • Masters Nursing Degree preferred

Responsibilities

  • Conduct comprehensive health assessments, including history and physical examinations
  • Provide preventative care, health screening and management of episodic illness
  • Diagnose, treat and prescribe in accordance with NP scope of practice
  • Identify, monitor, and manage acute and chronic conditions
  • Order and interpret diagnostic tests and reports (e.g. labs, imaging, ECGs)
  • Support health promotion and disease prevention for individuals and families
  • Deliver evidence-based care and communicate diagnoses effectively
  • Maintain professional competence through ongoing learning and development
