About The Position

Ultra Health Providers (UHP) is seeking a Nurse Practitioner (NP) to serve vulnerable populations through high-presence, quality care in skilled nursing and senior living communities. This role involves delivering onsite medical services to improve outcomes, reduce hospitalizations, and ensure timely, compassionate care. The NP will have clinical autonomy, engage in meaningful collaboration, and focus on patient-centered decision-making. UHP's mission is to serve patients, partners, and each other with tenacity, teamwork, and respectful confidence. The Nurse Practitioner will play a vital role in managing the medical needs of skilled nursing facility residents, acting as the primary point of contact for emergencies and urgent care visits, and working independently to see patients and treat complex medical conditions. This is a full-time position serving two facilities in Covington, TN and Ripley, TN.

Requirements

  • Active, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
  • Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
  • Clinical training and experience in adult, family, and/or geriatric medicine.
  • Eligibility to work in the United States.

Nice To Haves

  • Experience in internal medicine, geriatrics, cardiology, or post-acute care.
  • Active DEA registration or ability to obtain.
  • Strong clinical judgment and ability to work independently.
  • Excellent communication and interpersonal skills.
  • Organized, detail-oriented, and able to manage multiple priorities effectively.
  • Prior experience in SNF, LTC, ICU, or Emergency Department settings is a plus.
  • Ability to collaborate effectively within interdisciplinary care teams to optimize resident outcomes.

Responsibilities

  • Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
  • Independently evaluate, diagnose, and treat acute and chronic medical conditions.
  • Conduct initial admissions and follow-up assessments, including physical exams, medication review, and clinically appropriate screenings.
  • Monitor changes in resident condition and adjust treatment plans accordingly.
  • Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
  • Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
  • Communicate clearly and compassionately with residents and families regarding diagnoses, treatment plans, and changes in condition.
  • Maintain accurate and timely documentation in the electronic medical record in accordance with regulatory and payer standards.
  • Participate in quality and care coordination efforts to support excellent patient outcomes.
  • Adhere to HIPAA, state, and federal regulations while upholding ethical standards of clinical practice.
  • Verify admission orders and conduct initial assessment of residents.
  • Conduct Physical exams, medication review, and depression screening.
  • Collaborate with facility medical director, attending physicians and nursing staff to deliver the highest level of care to the patient.
  • Communicate with and counsel family members on treatment options when there are material changes in status.
