About The Position

Diagnostic Group is seeking a part-time Nurse Practitioner to join our medical team in Beaumont, Texas. This position primarily provides focused medical services to patients in their homes based on an identified clinical need or requested service. The Nurse Practitioner will perform comprehensive patient assessments, address the specific purpose of the visit, identify care needs or concerns, and communicate findings and recommendations to the patient’s physician or appropriate healthcare provider. This position is not intended to serve as the patient’s long-term primary care provider. While the primary focus of this position is home-based patient care, the Nurse Practitioner may also provide outpatient clinic and inpatient coverage as needed. Participation in weekend and holiday inpatient rotations is required. The ideal candidate will be highly organized, comfortable working independently, and possess strong clinical assessment, communication, and critical-thinking skills.

Requirements

Current and unrestricted Advanced Practice Registered Nurse (APRN) license, or equivalent advanced practice license, in the State of Texas.
Current applicable national board certification as a Nurse Practitioner.
Current Basic Life Support (BLS) certification required.
Valid driver’s license and reliable transportation required for patient home visits.
Strong physical assessment, clinical decision-making, and critical-thinking skills.
Ability to independently perform comprehensive and focused patient evaluations in a home setting.
Ability to identify and address specific clinical needs while maintaining continuity with the patient’s established healthcare providers.
Strong medication review and reconciliation skills.
Ability to recognize changes in patient condition and appropriately escalate care when necessary.
Strong organizational, time-management, communication, and documentation skills.
Proficiency with electronic health records (EHR).
Ability to work effectively across home-based, outpatient, and inpatient clinical settings.

Nice To Haves

Advanced Cardiovascular Life Support (ACLS) certification preferred.
Previous primary care, family medicine, internal medicine, home-based medical care, or inpatient experience preferred.

Responsibilities

Conduct comprehensive patient assessments, medical histories, and physical examinations in the patient’s home.
Provide focused evaluation and treatment based on the identified purpose or clinical need for the visit.
Assess acute and chronic medical conditions relevant to the requested service and address concerns within the scope of the visit.
Order appropriate laboratory studies, diagnostic testing, or other services when clinically indicated.
Prescribe medications or therapeutic treatments when appropriate and in accordance with Texas scope-of-practice requirements.
Perform medication review and reconciliation and identify potential medication-related concerns.
Provide preventive health assessments and assist with identifying and addressing applicable gaps in care.
Evaluate patients following hospitalization and identify concerns that may increase the risk of complications or readmission.
Provide patient and family education related to the patient’s current condition, medications, treatment recommendations, and follow-up needs.
Identify changes in patient condition and determine when additional evaluation or a higher level of care is appropriate.
Communicate visit findings, recommendations, and identified concerns to the patient’s physician or appropriate healthcare provider.
Coordinate referrals or follow-up services when clinically indicated.
Maintain timely, accurate, and complete documentation in the electronic health record (EHR).
Evaluate and treat patients with acute and chronic medical conditions in an outpatient clinic setting.
Provide routine and preventive care within the scope of the assigned clinic.
Order appropriate diagnostic testing and provide treatment based on clinical findings in an outpatient clinic.
Coordinate care with physicians, specialists, and other healthcare providers as appropriate in an outpatient clinic.
Assist Hospitalists with new admissions, hospital rounds, and discharges.
Evaluate and assist in the management of acute and chronic medical conditions in hospitalized patients.
Review laboratory results, imaging, medications, and other diagnostic information to assist with patient management.
Monitor patient progress and promptly communicate significant changes in condition to the Hospitalist.
Collaborate with physicians, nursing staff, case management, specialists, and other members of the multidisciplinary healthcare team.
Assist with discharge planning, medication reconciliation, patient education, and coordination of follow-up care.
