About The Position
This role involves providing daily responsibilities as a Nurse Practitioner Provider in a Skilled Nursing Facility. The position requires reviewing schedules, visiting patients, completing electronic charting, managing medications and treatments, and participating in meetings and on-call duties.
Requirements
- Experienced level.
- Graduate Degree.
- Must be able to complete electronic charting on patients within 24 hours of the visit.
- Must be able to sign each chart within 24 hours of each visit.
- Must be able to create a charge for each visit within 24 hours of each visit.
- Must be able to complete daily visit log for each patient.
- Must be able to send visit log to business office within 24 hours.
- Must be able to complete on-call log.
- Must be able to send on-call log to business office within 24 hours of completed on-call duties.
- Must submit hours, mileage, and PTO requests on a weekly basis to the business office.
Responsibilities
- Review schedule for assigned facility.
- Review scheduled appointments, check with Nursing Director for acute patient needs, and check with Medical Director for other needs.
- Add any new identified patient visits to the schedule and organize the day according to scheduled visits.
- Visit patients as scheduled, complete appropriate charting during the visit, and communicate with facility staff as needs arise.
- Prescribe and approve new medications, approve refill medications, order and approve labs, and order and approve new treatments as needed.
- Complete electronic charting on patients within 24 hours of the visit.
- Sign each chart within 24 hours of each visit.
- Create a charge for each visit within 24 hours of each visit.
- Complete a daily visit log for each patient, including name, date, and CPT code for each visit.
- Send the visit log to the business office within 24 hours.
- Participate in on-call duties as assigned.
- Complete an on-call log and send it to the business office within 24 hours of completed on-call duties.
- Participate in weekly provider meetings.
- Submit hours, mileage, and PTO requests on a weekly basis to the business office.
