About The Position

Humana's Primary Care Organization provides senior-focused, value-based care through its CenterWell and Conviva brands. The Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care works onsite at the Conviva Royal Palm Beach Center in Royal Palm Beach, Florida, as part of a collaborative multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and other care professionals. The role reports to the Associate Medical Director and provides comprehensive care to seniors in accordance with clinical standards. The position emphasizes coordinated, personalized care, with fewer patients per day to allow more time for meaningful patient care. The clinician works with the care team to support patients' physical, emotional, and social wellness, and contributes to value-based care and the center's growth plans. The role includes an on-call rotation.

Requirements

Master's degree in Nursing or completion of a Physician Assistant program with board certification.
Active, unrestricted Nurse Practitioner or Physician Assistant license in the state of practice.
Must undergo TB screening if selected for the role.

Nice To Haves

Experience in value-based care, primary care, or Medicare Advantage settings.
Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain a DEA license and/or Medicaid and Medicare numbers.
Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

Provide comprehensive care to seniors in accordance with clinical standards.
Collaborate daily with the care team to ensure coordinated, high-quality care.
Manage referrals, hospital and skilled nursing facility coordination, durable medical equipment, and home health services.
Document care accurately and efficiently, with support from quality-based coders.
Participate in the on-call rotation.
Support growth plans at the center.
Provide value-based care.
