About The Position
La Vita Bella Home Health and Hospice is seeking a compassionate, skilled, and dedicated Nurse Practitioner (PRN) to join our team. This role focuses on providing personalized care in patients' homes, building meaningful relationships, and making a lasting impact during life's most important moments. The Nurse Practitioner will assess patient needs, manage symptoms, collaborate with an interdisciplinary team, and help patients remain comfortable and supported at home. Requirements
- Active Nurse Practitioner (NP) license in the state of New Mexico
- National NP certification
- DEA license and prescriptive authority in New Mexico
- Strong clinical assessment, diagnostic, and communication skills
- Compassionate, calm, and patient-centered approach
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Reliable transportation and ability to travel within assigned territory
- A heart for serving others and making every moment matter
