Requirements

Graduate of an approved Nurse Practitioner curriculum, MSN required
Active APRN or NP license required
Minimum of two years clinical nursing experience
Valid driver’s license

Responsibilities

Conduct patient visits across all three service lines (Home Health, Hospice, and Palliative Care)
Collaborate and participate in weekly interdisciplinary group (IDG) meetings
Provide clinical guidance and continuity of care
Play a key role in shaping patient-centered outcomes
