Requirements

• Active New York State Nurse Practitioner license
• Current NYS Registered Nurse license in good standing
• DEA license required
• Master's Degree required
• Minimum 5 years of related clinical experience
• Current BLS certification

Nice To Haves

• ANCC certification preferred
• Experience in Substance Use, Addiction Medicine, Behavioral Health, or Community Health preferred
• Bilingual Spanish/English or Russian/English highly preferred

Responsibilities

• Conduct patient evaluations, assessments, and differential diagnoses
• Develop and implement treatment and care plans collaboratively with physicians and behavioral health teams
• Provide medication management and treatment recommendations
• Conduct waived testing including urine, blood work, and glucose monitoring
• Maintain accurate and confidential medical records
• Educate patients on treatment plans, health promotion, and recovery support
• Participate in interdisciplinary care coordination
• Lead educational groups and patient seminars
• Monitor patient progress and modify treatment plans as needed
