Nurse Practitioner

Circadia Health•New Jersey, NJ
•$140,000 - $180,000•Onsite

About The Position

Circadia Health is hiring an on-site Nurse Practitioner to provide patient care and help partner-facility teams apply predictive health signals to earlier interventions. The role combines direct assessments and care planning with hands-on training, workflow integration, and collaboration with facility providers and Circadia teams. The position is full-time and in the field, with a Monday–Friday presence at partner facilities in Central New Jersey. Travel may include sites within a 30–40-mile radius; mileage and lodging for sites outside that radius are covered per policy. The Nurse Practitioner will conduct initial and follow-up assessments for flagged residents, initiate or adjust plans of care, obtain consent, and coordinate with facility providers. They will interpret trend-based insights, prioritize high-risk patients, coach staff at the point of care, and help standardize Circadia use in clinical workflows. The role includes completing documentation on the same day, high-volume rounding (typically 30 or more visits per day where appropriate), providing field feedback on workflows and technology integration, and supporting NP onboarding and collaboration with clinical operations and product teams. Success measures include a target of 30 patient visits per day, at least 80% engagement, a time to acknowledgement under 24 hours, and completion of notes using AI-powered tools within 24 hours of each encounter. Adoption and impact are assessed through facility workflow adherence and a trend toward fewer falls and readmissions over time.
