About The Position
Chestnut Health is a multi-specialty healthcare organization providing rehabilitative care, with a focus on Physical Medicine and Rehabilitation (PM&R) and physiatry. The Nurse Practitioner will join a collaborative, patient-centered physiatry team and support patients with injuries, illnesses, or disabilities in restoring function, improving quality of life, and promoting independence. Requirements
- Active license to practice in the state of employment.
- Current Controlled Substance License, required upon start date.
- Current DEA License, required upon start date.
- ANCC Certificate or AANP Certificate.
- Certified Nurse Practitioner certification.
- Desire to work in Physiatry/Physical Medicine and Rehabilitation.

Responsibilities
- Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
- Conduct patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
- Monitor patient progress and adjust treatment plans as needed to optimize functional outcomes and promote independence.
- Participate in multidisciplinary team meetings, case conferences, and educational activities.
- Maintain accurate and timely electronic medical record documentation, including initial evaluations, treatment plans, progress notes, and discharge summaries.
- Educate and counsel patients and their families about the rehabilitation process, expected outcomes, and post-discharge care.
