About The Position

Requirements

  • Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited nurse practitioner program.
  • Possess a valid and unrestricted nurse practitioner license in the State of Florida.
  • Be certified as a nurse practitioner by a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Demonstrate proficiency in conducting physical assessments, diagnosing medical conditions, and developing treatment plans.
  • Have excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare team members.
  • Bilingual in Spanish is required.

Responsibilities

  • Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results.
  • Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed.
  • Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling.
  • Educate patients and their families about health conditions, treatment options, medications, and self-care techniques.
  • Collaborate with the Medical Director, Operations Manager, Care Navigators, and Medical Assistants to streamline patient care.
  • Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform.
  • Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards.
  • Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed.
  • Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines.
  • Support CHI initiatives aimed at reducing healthcare disparities in underserved communities.
  • Ensure that patients receive appropriate follow-up care, either personally or through coordinated care teams.
