About The Position

Parker At Your Door, a medical house calls program serving the greater New York Metro region, is seeking a Primary Care Nurse Practitioner to provide quality primary medical care to patients in their homes in Suffolk County. The provider assesses community-dwelling members with multiple comorbidities, performs comprehensive patient evaluations, and provides medical management. This is a key clinical role in a growing community program, offering the opportunity to make an impact within a health care organization.

Requirements

• Licensure and current registration to practice as a Nurse Practitioner in New York State.
• Strong communication and follow-up skills.
• Knowledge of Microsoft applications, including Excel, and of data entry, documentation, and report generation.

Nice To Haves

• Prior experience with the geriatric population and home care is strongly preferred.
• Knowledge of medical documentation.

Responsibilities

• Examine patients and order appropriate diagnostic workups.
• Assess members experiencing an acute change in condition in a timely manner.
• Maintain complete and accurate medical records.
• Consult with interdisciplinary team members to create appropriate and viable care plans.
• Educate patients about disease processes, medications, and compliance.
• Provide medical management of patients.
