About The Position

Delivers primary health care services to patients of all ages, with a significant emphasis on the pediatric population. Provides individual and/or group health education and counseling while providing outreach to and advocacy for the homeless population.

Requirements

  • Must be licensed as a Registered Nurse Practitioner in the State of Kentucky
  • Successful completion of a Nurse Practitioner training program in an accredited educational institution
  • At least two years of broad-based professional nursing experience
  • Must have a valid driver’s license
  • Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency.

  • Willing to travel the service area as needed. Travel is primarily local or within the service area during the business day, although some out-of-area and overnight travel may be expected.

Responsibilities

  • Provides comprehensive primary care services to patients of all ages, with a strong focus on pediatric patients.
  • Delivers primary health care services to patients in the clinic and in the mobile healthcare unit.
  • Provides COVID services as needed in the clinic and in the mobile healthcare unit.
  • Provides individual and/or group health education and counseling while providing outreach to and advocacy for the homeless population.
  • Conducts physical examinations of patients.
  • Exercises sound nursing judgment when initiating and prescribing therapy within the guidelines of written protocols.
  • Interviews patients, documents their medical history and makes assessments and/or physical diagnosis of condition.
  • Maintains proper clinical records and reports, using problem-oriented medical records which can be audited for accountability.
  • Participates in program Quality Assurance (QA) committee and quality improvement efforts.
  • Performs and completes PCMH duties as assigned. Duties include but are not limited to: attending patient huddle, timely clinical advice, involved in QI activities, and access needs.
  • Initiates medications and treatment within the scope of practice.
  • Supervises assigned clinical staff.
  • Refers patients for specialized medical care beyond the scope of the HCH Services, including arrangement for any necessary ambulance transportation.
  • Collaborates with the Clinical Director in review/revision of policies, protocols, and procedures as necessary.
  • Responsible for covering after-hour calls for FHWC.
  • Participates in and/or conducts in-service presentations.
  • Performs all other duties as assigned.
