About The Position

The Client is a pioneering medical group focused on the needs of patients with chronic kidney disease (CKD). Our Health Mobilization™ platform brings together multidisciplinary care teams with community nephrologists, empowering a holistic approach to patient care. The Nurse Practitioner will be an integral part of a care team, managing a small panel of patients with CKD stages 3b, 4, 5, and ESRD. This role involves direct patient interactions in various settings, including home visits, teleconferences, and phone consultations. The NP will work closely with the care team to provide high-quality, value-based care, focusing on both patient-level and system-level interventions.

Requirements

• Licensed Advanced Practice Nurse (APN) in the state of Arizona.
• Must meet Client clinician credentialing criteria, including background check, drug screen, and COVID-19 vaccine requirements.
• Reliable transportation and a verifiable good driving record.
• Reside in the Phoenix area and be able to travel to patient homes.
• Self-motivated with the ability to follow defined operating procedures.
• Strong ability to build inclusive relationships with patients, caregivers, and staff.
• Experience working with diverse community resources.
• Excellent communication skills, both oral and written.
• Comfortable with home visits to patients.

Responsibilities

• Provide clinical care for an assigned panel of patients in collaboration with the care team.
• Conduct comprehensive history and physical exams, and order necessary laboratory and diagnostic tests.
• Educate patients about their medications, manage prescriptions, and monitor for side effects.
• Identify and address medical conditions and barriers to care.
• Participate in care team huddles and contribute to patient care planning.
• Facilitate care across various settings, including hospitals, skilled nursing facilities, and community organizations.
• Perform structured assessments and develop comprehensive care plans.
• Offer after-hours telephonic support as needed.
• Document all patient encounters accurately and promptly.
• Assist in the development and implementation of clinical protocols to improve outcomes.
• Maintain patient confidentiality and adhere to all company policies.
