About The Position
The Nurse Practitioner (VSP) applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner (VSP) works on problems of diverse scope and complexity ranging from moderate to substantial. This is a variable service provider NP/PA (APP) role with an average of 20-30 hours per week. Licensure preferred in TX, AZ, VA, FL, MS. This is a fully remote position.
Requirements
- Master's in science in Nursing
- Active, unrestricted NP Licensure preferred in TX, AZ, MS, VA, AND FL.
- 2+ years of experience
Responsibilities
- Call Humana members to discuss open care gaps and placing needed orders (specifically women who had a fracture in the last 6 months)
- Create and register a patient record within Athena
- Verify benefits
- Gather medication, medical and surgical history
- Coordinate management of members social needs to help facilitate gap closure – providing transportation services.
