About The Position

David Raines Community Health Centers is looking for passionate, qualified applicants to join our team! The Family Medical Nurse Practitioner works under the supervision of the Medical Director and in close coordination with the other Health Center personnel. The Family Medical NP provides medical care to all clinic patients, to include examination, diagnosis and treatment, with referrals to specialists or hospitals where indicated.

Requirements

  • Master of Science Degree in Nursing or Doctor of Nursing Practice with family nurse practitioner focus.
  • Minimum 2 years of experience in healthcare preferred.
  • Excellent interpersonal and communication skills.
  • Experience with electronic health records required, preferably Next Gen or similar EHS.
  • Strong clinical judgement and decision-making skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Commitment to continuing education and professional development.
  • Must be licensed as a Nurse Practitioner to practice in the State of Louisiana.
  • Valid FNP certification from AANP or ANCC
  • Current BLS (American Heart Association)
  • Active DEA licensure

Responsibilities

  • Conduct comprehensive physical exams, health screenings and assessments.
  • Diagnose and treat acute and chronic conditions.
  • Administer vaccinations and other preventive healthcare measures.
  • Provide education and support to families on health, wellness, and disease prevention.
  • Order and interpret diagnostic tests, such as lab work and imaging studies.
  • Prescribe medications and therapies, including controlled substances within their scope of practice.
  • Monitor and manage patient progress, adjusting care plans as needed.
  • Maintain accurate and detailed electronic medical records (EMR) within a timely manner and collaborate with physicians/specialists.
  • Advise and participate in the development of family medicine healthcare policies and practices.
  • Participate in community outreach programs to promote health and wellness.
  • Meets with other members of the staff and participate in planning and implementing improvements to health services.
  • Frequent contact is maintained with the Medical Director for whom he/she is responsible.
  • Adheres to the standards of medical practice; is responsible for treatment diagnosis, referrals, consultation and assist in medical education for both patients and center staff.
  • Perform other duties as required.
